Mistakes and medical errors

 Everyone makes mistakes. And as healthcare professionals we make mistakes as well.  Some mistakes are small, some mistakes are serious. Some serious mistakes are a system error rather than an individual doctors mistake. I am going to explain all mistakes from a low intensity to the worst case scenarios in this blog post.  DATIX  This system is to identify any preventable errors in the work place.  The person who has identified such issues has to fill in a form online and then submit it.  Datixes can range from : 1. Fall on the ward - the nurses usually fill it  2. Drug error - any one can do it. Usually the pharmacists are good at doing this.  3. Patient care - an inappropriate CPR  4. Taking referrals as a medical registrar

  As a medical registrar on call, we get referrals from different specialties all the time.  I am going to go through a few bleeps I get when I am on call.  I am going to share some tips in general: 1. Never mock the other person down the phone.  2. Write down whatever they are saying- summarize the case quickly at the end to ensure you have the relevant information.   3. If they are worried, see the patient ASAP  4. Give urgent management advice on the phone - like request them to do an ABG, etc  5. Understand their limitations. They may not have the same medical experience as you do.  6. Working as a junior doctor on AMU - what is expected from you

Working on AMU can be a daunting experience for many.
